New Patient Evaluation
All new patients must have an evaluation to begin physical therapy. Doctor referral not required.
Service Description
Whether you are new to Therapy323 or a returning patient with a new injury, an evaluation is required to begin physical therapy. You do not have to get a referral from your doctor before your visit; we can request a referral from your doctor after your initial eval. Direct access to treatment and less work for you.
Cancellation Policy
Patient is financially responsible for all services provided by Therapy323 and payment is due at the time of service. Patient must provide at least 24 hours notice to cancel or reschedule an appointment. NOT providing 24 hours notice will result in being charged for the appointment. Repeated missed appointments may result in additional fees or discharge from care. We accept BlueCross BlueShield of Texas insurance and Medicare, but that does not guarantee coverage. Patient is responsible for copays or coinsurance at the time of the appointment. If insurance denies a claim, patient is responsible for payment. For other insurance companies, a superbill may be requested for the patient to submit to their insurance for possible reimbursement (not guaranteed). Reimbursement typically depends on your insurance's out of network benefits.
